IMPORTANT-

PLEASE READ TO COMPLY WITH FILING DATES AND REQUIREMENTS

 

Applications must be filled out completely. We will not accept incomplete applications.

All questions must be answered in detail. 

FERMENTED MALT BEVERAGE AND INTOXICATING LIQUOR LICENSES, Forms AT-115 and AT-106:  

  • Corporations must apply for licenses under the name for which they are registered with the Secretary of State's Office.

 

  • A Corporation must have at least two officers sign the application; namely a President and a Secretary who are not the same person.

 

  • The President and Vice-president of the corporation also cannot be the same person.

 

  • Every Corporation must have an appointed Agent.

 

  • Signatures must be notarized.

 

  • Make sure that applications are complete.  

Furnish a complete description of the licensed premise (rooms, buildings and/or land area) where alcoholic beverages are to be sold, served and stored. If your description is too long for the space provided on the form please use a separate piece of paper for the whole premise description and attach to the renewal application. PREMISE DESCRIPTIONS WILL NOT BE CARRIED FROM PREVIOUS YEAR. LICENSES WILL BE ISSUED FOR THE PREMISE DESCRIPTION PROVIDED ON APPLICATION.   

PLEASE NOTE: Renewal form AT-115 or Original form AT-106 requires your Wisconsin Seller's Permit number and your Federal Employer Identification Number.  

If there has been a change in ownership you will need to complete: 

AT- 103 Auxiliary Questionnaires
AT- 104 Agent Schedule
AT- 106 Original Alcohol Beverage License Application

 

If there has been a change in officers of a corporation you will need to complete: 

AT- 103 Auxiliary Questionnaires

 

If there has been a change in Agent you will need to complete: 

AT- 103 Auxiliary Questionnaires
AT- 104 Agent Schedule

FILING DATES:

Applications for Fermented Malt Beverage/Intoxicating Liquor Licenses are due on or before April 15th 2011. 

Applications for Fermented Malt Beverage and Class C-Wine Licenses are due on or before May 13th, 2011. 

Class C-Wine applications must also include sales figures for the previous year.
