As of July 1, 2008  you must file monthly. 

Unfillable form is to print and fill in by hand, make sure you print two copies to fill in.

The fill in form you can type in your information on the pdf, and print 2 completed forms.
You can save the fill in form to your computer for quick use next time, but you will not be able to save the information typed in the form unless you have Adobe Acrobat.

Send to the Village of Lake Delton and retain one copy for your records.

This form needs to filled out and sent to the Village of Lake Delton before you are permitted to collect 5% Room Tax.

As of July 1, 2008 the Quarterly Report will no longer be valid except to file for 2nd Quarter 2008 due by July 31, 2008. Please use Monthly Reporting Form above.
