PARTICIPATION GUIDELINES
To all whom will be participating in the Kops 4 Kids program, or receiving the completed forms Please read below:
1. All family request forms are due by December 6th. These forms may be picked up at either the Lake Delton or Wisconsin Dells Police Departments or available online at www.wdkopsforkids.com
2. All forms must be completed entirely. Failure to do so may result in disqualification from the program.
3. The children must reside in the WI Dells School District.
4. Children who have signed up at other organizations such as “Kops 4 Kids” will not be able to participate in this program. We compare lists of names, and you will be disqualified.
5. Age Limit: Children who have not yet obtained their 13th birthday at the time the form is completed may participate. NO Adults will be considered this year.
6. The party is by invitation only. Meaning, only those children that are invited may attend, along with that child’s parent or guardian. Do not bring other children, as they will not be allowed inside. Space is very limited, and this is a organized and donated event.
7. Attendance of the invited child is REQUIRED to receive presents. No exceptions.

SCHEDULE OF EVENTS
December 6 - all family request forms are due. The forms will be available at the WI Dells and Lake Delton PD, along with www.wdkopsforkids.com
December 11 – Donation/Gift drive. Please go to Lake Delton Wal-Mart between 10am and 4pm and make a donation.
December 21 – Kops 4 Kids party, by invitation only
