Citizen Report of Incident
If you have an incident you would like to report involving a minor property crime with no suspects, that you did not directly report to an officer, you may print out the following form, fill it out and fax it to us.

The following form can be used to submit your most current business contact information to have on file with the Lake Delton Police Department.  You can fill out the form and email it to ldpdclerk@lakedelton.org or fax it to 254-4651.
